# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of nasal mucosal blood vessels. This reduces swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended without physician guidance. Safety and efficacy not established.
## Dose Adjustments
No dose adjustment is typically required based on renal or hepatic impairment for topical nasal use.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use in patients with narrow-angle glaucoma.
* Use in patients using MAO inhibitors or within 2 weeks of discontinuing MAO inhibitors.
## Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing, transient nasal irritation.
Less Common/Serious: Rebound congestion (rhinitis medicamentosa) with prolonged or excessive use, hypertension, cardiac arrhythmias, headache, insomnia, nervousness.
## Key Drug Interactions
* **MAO Inhibitors:** Concomitant use or recent use can lead to a hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effect of oxymetazoline.
## Monitoring
* Monitor for signs of rebound congestion, especially with use exceeding the recommended duration.
* Monitor blood pressure and heart rate in patients with underlying cardiovascular conditions, particularly with frequent or prolonged use.
## Clinical Pearls
* Limit use to a maximum of 3 days to prevent rebound congestion (rhinitis medicamentosa).
* If congestion is not relieved, consider alternative treatments and consult a healthcare provider.
* Prime the pump mechanism before first use or if not used for an extended period.
