## Sinex (Oxymetazoline)
### Overview
Sinex is a nasal spray containing oxymetazoline, an alpha-adrenergic agonist that causes vasoconstriction of nasal mucosal blood vessels, reducing swelling and congestion.
### Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
### Adult Dosing
* **Nasal Spray (0.05%):** 2 sprays in each nostril every 10-12 hours. Do not exceed 2 applications in a 24-hour period.
### Pediatric Dosing
* **Children 6 years and older:** 2 sprays in each nostril every 10-12 hours. Do not exceed 2 applications in a 24-hour period.
* **Children under 6 years:** Safety and efficacy not established. Consult a pediatrician.
### Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or within 14 days of stopping a monoamine oxidase inhibitor (MAOI).
### Adverse Effects
* **Common:** Nasal discomfort, stinging, burning, dryness, sneezing.
* **Less Common:** Rhinitis medicamentosa (rebound congestion) with prolonged or excessive use, increased blood pressure, palpitations, insomnia, nervousness.
### Key Drug Interactions
* **MAOIs:** Concomitant use or recent use (within 14 days) can cause severe hypertensive crisis.
* **Other sympathomimetic agents:** May potentiate pressor effects.
### Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged use.
* Monitor blood pressure in patients with pre-existing hypertension, especially with frequent or prolonged use.
### Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* If nasal passages remain blocked, consider alternative therapies or consulting a healthcare provider.
* Proper nasal spray technique is important: tilt head slightly forward, insert tip into nostril, spray while sniffing gently, and hold head over sprayed nostril for a moment.
