# Sinex (oxymetazoline)
## Overview
Sinex is a nasal decongestant that works by constricting blood vessels in the nasal passages, reducing swelling and improving airflow. It is available as a nasal spray.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray (0.05%):** Prime pump by spraying 4 times. Administer 2-3 sprays into each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Ages 6-11 years:** 1-2 sprays into each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Under 6 years:** Safety and efficacy have not been established. Use is not recommended.
## Dose Adjustments
* No dose adjustment is typically needed for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken MAO inhibitors within the past 14 days.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Less Common:** Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, insomnia, nervousness, increased blood pressure.
## Key Drug Interactions
* **MAO Inhibitors (e.g., phenelzine, tranylcypromine):** Concurrent use can lead to hypertensive crisis. Avoid use within 14 days of MAO inhibitor therapy.
* **Other sympathomimetic agents:** May potentiate cardiovascular effects such as increased blood pressure and heart rate.
## Monitoring
* Monitor for rebound congestion with prolonged use (beyond 3 days).
* Assess for signs and symptoms of systemic absorption, especially in patients with cardiovascular disease or hypertension.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* If symptoms persist, consult a healthcare provider.
* Ensure the nasal spray pump is primed before first use and if not used for an extended period.
* Not intended for continuous or long-term use.
