### Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of nasal mucosal vessels. This reduces swelling and congestion.
### Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
### Adult Dosing
* **Standard:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
### Pediatric Dosing
* **Ages 6 to <12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Ages <6 years:** Safety and efficacy not established. Consult a pediatrician.
### Dose Adjustments
* No dose adjustments are typically required based on renal or hepatic impairment for topical nasal use, given minimal systemic absorption.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Concurrent use with MAO inhibitors or within 2 weeks of discontinuing MAO inhibitors (risk of hypertensive crisis).
* Patients with narrow-angle glaucoma.
### Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, sneezing, increased nasal discharge.
* **Rebound Congestion (Rhinitis Medicamentosa):** Occurs with prolonged or excessive use (>3 days), leading to worsening congestion upon cessation.
* **Systemic Effects (Rare with topical use):** Hypertension, palpitations, headache, nervousness, insomnia.
### Key Drug Interactions
* **MAO Inhibitors (MAOIs) and within 14 days of MAOI discontinuation:** Potential for hypertensive crisis.
* **Other Vasoconstrictors:** May potentiate systemic hypertensive effects.
### Monitoring
* Monitor for signs of rebound congestion (worsening congestion with continued use).
* Monitor blood pressure in patients with pre-existing hypertension, especially with frequent or prolonged use, although systemic absorption is generally low.
### Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* If symptoms persist beyond 3 days, discontinue use and consult a healthcare provider.
* Blow nose gently before administration.
* Shake the container before each use.
