# Oxymetazoline Nasal Spray
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical vasoconstrictor.
## Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Daily Dose:** Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended without physician advice.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or who have taken MAO inhibitors within the last 14 days.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, sneezing, increased nasal discharge. Rebound congestion (rhinitis medicamentosa) with prolonged use.
* **Less Common:** Dizziness, headache, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **MAO Inhibitors:** Concomitant use can potentiate the hypertensive effects of oxymetazoline. Avoid use within 14 days of MAO inhibitor discontinuation.
* **Other Sympathomimetics:** May increase the risk of cardiovascular side effects.
## Monitoring
* Monitor for rebound congestion with prolonged or frequent use (typically > 3 days).
* Assess blood pressure and heart rate, especially in patients with pre-existing cardiovascular conditions or those using other sympathomimetics.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* Instruct patients to avoid spraying into the eyes.
* If symptoms persist after 3 days of use, discontinue and consult a healthcare provider.
