## Overview
Sinex (oxymetazoline) is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of nasal blood vessels. This reduces swelling and congestion in the nasal passages.
## Primary Indications
* Nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Maximum Duration of Use:** Limit use to a maximum of 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Children under 6 years:** Use is not recommended. Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment for topical nasal use.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients who have undergone recent nasal surgery.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, sneezing.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, insomnia, nervousness, increased blood pressure, palpitations, arrhythmias.
## Key Drug Interactions
* **MAO Inhibitors (and within 14 days of stopping):** Concomitant use can lead to a hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
* **Beta-blockers:** May blunt the hypotensive effects and lead to increased peripheral resistance.
## Monitoring
* Monitor for signs of rebound congestion with prolonged or frequent use.
* Monitor blood pressure and heart rate, especially in patients with cardiovascular disease or those taking interacting medications.
## Clinical Pearls
* For temporary relief of nasal congestion only.
* Avoid spraying in the eyes.
* Educate patients on the importance of limiting use to 3 days to avoid rebound congestion.
