# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, leading to decongestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** Apply 2-3 sprays into each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Nasal Spray:** Not recommended for use in children under 6 years of age without medical advice. For children 6 years and older, follow adult dosing.
## Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients using or who have used MAO inhibitors within the past 14 days.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, dryness, sneezing, increased nasal discharge.
* **Rebound Congestion (Rhinitis Medicamentosa):** This is a significant risk with prolonged or excessive use (typically > 3-5 days). Symptoms include worsening congestion upon cessation of the drug.
* **Systemic effects (rare with proper use):** Headache, nervousness, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use can lead to a hypertensive crisis. Avoid use within 14 days of MAO inhibitor therapy.
* **Other Vasoconstrictors:** May potentiate pressor effects. Use with caution.
## Monitoring
* Monitor for rebound congestion with prolonged use.
* Monitor blood pressure in patients with pre-existing hypertension or cardiovascular disease, especially with frequent use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to minimize the risk of rebound congestion.
* If symptoms persist or worsen, discontinue use and consult a healthcare provider.
* Proper nasal spray technique is important: tilt head slightly forward, insert nozzle into nostril, and spray while breathing in gently.
