# Sinex (Oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, a direct-acting alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Sinex Original/Sinex SE:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Sinex Longer Lasting:** 2 sprays in each nostril every 12 hours as needed. Do not exceed 2 doses in 24 hours.
## Pediatric Dosing
* **Children 6 to under 12 years:** Use Sinex Original/SE or Sinex Longer Lasting. 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Children under 6 years:** Consult a pediatrician or healthcare provider.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment. However, use with caution in patients with these conditions due to potential systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or have taken MAO inhibitors within the past 14 days.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, dryness, sneezing, runny nose.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use (>3 days), headache, dizziness, insomnia, increased blood pressure, palpitations, nervousness.
## Key Drug Interactions
* **MAO Inhibitors:** Potentially severe hypertensive crisis. Avoid concomitant use.
* **Other Sympathomimetic Agents:** May potentiate cardiovascular effects (e.g., increased blood pressure, heart rate).
## Monitoring
* Duration of use: Advise patients to limit use to no more than 3 consecutive days to avoid rebound congestion.
* Blood pressure and heart rate: Monitor in patients with cardiovascular disease or hypertension, especially with frequent or prolonged use.
## Clinical Pearls
* Rebound congestion is a significant concern with prolonged use. Patients should be advised to discontinue use if symptoms persist and consult a healthcare provider.
* Systemic absorption can occur, particularly with overuse, potentially leading to cardiovascular effects. Use caution in patients with underlying cardiovascular conditions.
