# Sinex (oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, a direct-acting alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** Prime the pump before first use. Administer 2 sprays into each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 spray into each nostril every 10-12 hours.
* **Children under 6 years:** Safety and efficacy not established. Consult a pediatrician.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment. However, caution is advised in patients with these conditions due to potential systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients who have undergone recent transsphenoidal hypophysectomy or other nasal/intranasal surgery where dura mater exposure has occurred.
## Adverse Effects
* **Common:** Nasal irritation, dryness, burning, sneezing.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic effects (hypertension, tachycardia, palpitations, nervousness, insomnia, headache) especially with overuse or in susceptible individuals.
## Key Drug Interactions
* **MAO Inhibitors (and within 2 weeks of stopping):** Potential for hypertensive crisis.
* **Beta-blockers:** May potentiate hypertensive effects.
* **Tricyclic Antidepressants:** May potentiate hypertensive effects.
## Monitoring
* Monitor for signs of rebound congestion or systemic side effects, especially with prolonged or excessive use.
* Blood pressure and heart rate may be monitored in patients with cardiovascular disease or those experiencing systemic symptoms.
## Clinical Pearls
* Limit use to 3 days to prevent rhinitis medicamentosa.
* Prime the pump before each use.
* Advise patients to avoid spraying into the eyes.
* Use with caution in patients with heart disease, hypertension, diabetes, thyroid disease, or enlarged prostate.
