# Sinex (oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
**Nasal Spray:**
Instill 2 to 3 sprays in each nostril every 10 to 12 hours.
Do not exceed 2 doses in a 24-hour period.
Limit use to a maximum of 3 consecutive days.
## Pediatric Dosing
**Nasal Spray:**
Children 6 years and older: Instill 2 to 3 sprays in each nostril every 10 to 12 hours.
Do not exceed 2 doses in a 24-hour period.
Limit use to a maximum of 3 consecutive days.
Children under 6 years: Not recommended. Consult a healthcare professional.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment. However, caution should be exercised in patients with cardiovascular disease, hypertension, hyperthyroidism, diabetes mellitus, or prostatic hyperplasia.
## Contraindications
Hypersensitivity to oxymetazoline or any component of the formulation.
Patients who have undergone transsphenoidal hypophysectomy or nasal/eye surgery with dural exposure.
## Adverse Effects
Common: Nasal burning, stinging, sneezing, increased nasal discharge.
Less Common: Rhinitis medicamentosa (rebound congestion), headache, dizziness, insomnia, palpitations, hypertension.
## Key Drug Interactions
Use with MAO inhibitors or within 2 weeks of discontinuing MAO inhibitors may cause hypertensive crisis.
Concurrent use with other sympathomimetic agents may increase blood pressure and heart rate.
## Monitoring
Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged use.
Monitor blood pressure, especially in patients with pre-existing hypertension or cardiovascular disease.
## Clinical Pearls
Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
If symptoms persist or worsen, discontinue use and consult a healthcare professional.
Avoid accidental ingestion.
