# Sinex (oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, an alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* Administer 2-3 sprays into each nostril every 10-12 hours.
* Do not exceed 2 doses in a 24-hour period.
* Limit use to a maximum of 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years of age:** 2-3 sprays into each nostril every 10-12 hours. Do not exceed 2 doses in a 24-hour period. Limit use to a maximum of 3 consecutive days.
* **Children under 6 years of age:** Consult a healthcare provider. Safety and efficacy have not been established.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment, but caution is advised.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Avoid use in patients with conditions where vasoconstriction could be harmful (e.g., severe hypertension, cardiovascular disease, hyperthyroidism, diabetes, prostatic hypertrophy).
## Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing, increased nasal discharge, headache, dizziness, nausea.
Less Common/Serious: Rebound congestion (rhinitis medicamentosa) with prolonged use, cardiovascular effects (hypertension, tachycardia, palpitations), CNS stimulation, insomnia.
## Key Drug Interactions
* **MAO inhibitors and tricyclic antidepressants:** Concurrent use may potentiate the pressor effects of oxymetazoline. Discontinue MAO inhibitors at least 14 days before starting oxymetazoline.
* **Beta-blockers:** May mask the reflex tachycardia caused by oxymetazoline.
## Monitoring
* Monitor for signs of rebound congestion (increased nasal stuffiness) upon discontinuation.
* Monitor for systemic side effects, especially in patients with cardiovascular disease or hypertension.
## Clinical Pearls
* Emphasize the importance of limiting use to 3 days to prevent rhinitis medicamentosa.
* Instruct patients on proper nasal spray technique to ensure optimal delivery and minimize systemic absorption.
* Advise patients to consult a healthcare provider if symptoms persist or worsen.
