# Sinex (oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Dose:** Do not exceed 2 applications in a 24-hour period. Do not use for more than 3 consecutive days.
## Pediatric Dosing
* **Children 6 years and older:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Consult a healthcare professional.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, but use with caution and consider systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Use within 14 days of discontinuing a monoamine oxidase inhibitor (MAOI).
## Adverse Effects
* **Common:** Nasal discomfort, dryness, burning, sneezing, stinging, increased lacrimation.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic effects (hypertension, nervousness, insomnia, palpitations, headache, dizziness) especially with overuse or in sensitive individuals.
## Key Drug Interactions
* **MAOIs (and within 14 days of stopping):** Potentially life-threatening hypertensive crisis.
* **Other sympathomimetic agents:** May potentiate cardiovascular effects.
## Monitoring
* Assess for relief of nasal congestion.
* Monitor for adverse effects, particularly rebound congestion and signs of systemic absorption.
* Advise patients to limit use to 3 days to avoid rebound congestion.
## Clinical Pearls
* Prolonged or excessive use can lead to rhinitis medicamentosa (rebound congestion), making nasal congestion worse.
* Instruct patients on proper spray technique to avoid excessive systemic absorption.
* Use caution in patients with cardiovascular disease, hypertension, hyperthyroidism, diabetes mellitus, or prostatic hyperplasia.
