# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of the nasal mucosa. This reduces swelling and congestion.
## Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
Administer 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in a 24-hour period.
## Pediatric Dosing
* **Children 6 years and older:** Administer 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in a 24-hour period.
* **Children under 6 years:** Safety and efficacy have not been established. Consult a healthcare provider.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment for topical nasal application.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Patients who have undergone transsphenoidal hypophysectomy or other transnasal surgery with dural exposure.
## Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing.
Less Common: Rhinitis medicamentosa (rebound congestion) with prolonged use (>3 days), palpitations, increased blood pressure, dizziness, headache, insomnia.
## Key Drug Interactions
Concomitant use with MAO inhibitors or within 2 weeks of discontinuing MAO inhibitors may cause hypertensive crisis. Use with caution in patients taking beta-blockers or other antihypertensives.
## Monitoring
Monitor for signs of rebound congestion (rhinitis medicamentosa), particularly with prolonged or frequent use. Assess blood pressure, especially in patients with pre-existing hypertension or cardiovascular disease.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* Proper technique involves aiming the spray away from the nasal septum.
* If nasal passages become occluded, advise the patient to wait 10 minutes before using again.
