# Sinex (oxymetazoline) Nasal Spray
## Overview
Sinex is an over-the-counter nasal spray containing oxymetazoline, a topical decongestant. It works by constricting blood vessels in the nasal passages, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Standard:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days.
## Pediatric Dosing
* **Ages 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Ages under 6 years:** Consult a healthcare provider.
## Dose Adjustments
* No specific dose adjustments are recommended for renal or hepatic impairment due to minimal systemic absorption. However, caution is advised in patients with certain medical conditions (see Contraindications).
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or have within the past 2 weeks taken monoamine oxidase inhibitors (MAOIs).
* Risk of rebound congestion and rhinitis medicamentosa with prolonged use.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Less Common:** Runny nose, headache, dizziness, insomnia, palpitations, increased blood pressure.
* **Serious:** Rebound congestion, rhinitis medicamentosa, systemic absorption leading to cardiovascular effects (hypertension, tachycardia).
## Key Drug Interactions
* **MAOIs:** Concomitant use or use within 2 weeks of MAOIs can lead to severe hypertension.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effect of oxymetazoline.
* **Beta-blockers:** May reduce the effectiveness of beta-blockers and increase the risk of reflex bradycardia.
## Monitoring
* Monitor for signs of rebound congestion or rhinitis medicamentosa.
* Monitor blood pressure, especially in patients with pre-existing hypertension or cardiovascular disease.
## Clinical Pearls
* Limit use to a maximum of 3 days to prevent rebound congestion and rhinitis medicamentosa.
* If symptoms persist after 3 days, advise the patient to discontinue use and consult a healthcare provider.
* Avoid spraying directly onto the nasal septum.
