# Sinex (oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, a topical decongestant. It works by constricting blood vessels in the nasal passages, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** Prime the pump before first use by squirting it into the air until a fine mist appears. Insert the spray nozzle into one nostril and spray 2-3 times. Inhale gently through the nose. Repeat in the other nostril.
* Use every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 doses in a 24-hour period.
* **Duration of Use:** Do not use for more than 3 consecutive days.
## Pediatric Dosing
* **Children 6 to under 12 years:** 2-3 sprays in each nostril every 10-12 hours. Do not exceed 2 doses in a 24-hour period. Do not use for more than 3 consecutive days.
* **Children under 6 years:** Consult a healthcare professional.
## Dose Adjustments
No dose adjustments are typically needed for renal or hepatic impairment due to minimal systemic absorption. However, caution is advised in patients with pre-existing cardiovascular conditions.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use in patients with narrow-angle glaucoma.
* Concurrent use with monoamine oxidase inhibitors (MAOIs) or within 2 weeks of discontinuing MAOIs.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, dryness, sneezing, transient nasal irritation.
* **Rebound Congestion (Rhinitis Medicamentosa):** Prolonged or excessive use can lead to worsening congestion. This is the most significant concern with overuse.
* **Systemic Effects (rare with nasal use):** Increased blood pressure, headache, nervousness, insomnia, palpitations.
## Key Drug Interactions
* **MAOIs and other sympathomimetics:** Concomitant use can lead to hypertensive crisis. Avoid use within 2 weeks of MAOI discontinuation.
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa), especially with prolonged use.
* Monitor blood pressure if systemic absorption is suspected, particularly in sensitive individuals.
## Clinical Pearls
* Sinex is for symptomatic relief of nasal congestion and does not treat the underlying cause of the congestion.
* Adherence to the maximum daily dose and duration of use (3 days) is crucial to prevent rebound congestion.
* If symptoms persist or worsen after 3 days, advise the patient to discontinue use and consult a healthcare professional.
* Proper priming of the nasal spray is important for consistent dosing.
