## Oxymetazoline Nasal Spray
### Overview
Oxymetazoline is an alpha-adrenergic agonist that causes vasoconstriction of nasal mucosal blood vessels, reducing swelling and congestion.
### Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
### Adult Dosing
* **2 sprays in each nostril every 10-12 hours as needed.**
* **Maximum duration of use: 3 days.** Prolonged use can lead to rebound congestion (rhinitis medicamentosa).
### Pediatric Dosing
* **Children 6 to 11 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed. Maximum duration of use: 3 days.
* **Children under 6 years:** Use is not recommended or should be under the guidance of a healthcare provider. Specific dosing is not well-established for this age group.
### Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, but prolonged use should be avoided in all patients.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Use with caution in patients with cardiovascular disease (e.g., hypertension, ischemic heart disease), hyperthyroidism, diabetes mellitus, or prostatic hyperplasia.
### Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Less Common:** Rebound congestion (rhinitis medicamentosa) with prolonged use, increased blood pressure, palpitations, insomnia, nervousness, headache.
### Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs) and within 14 days of stopping MAOIs:** Concomitant use may potentiate the pressor effect of oxymetazoline.
* **Other sympathomimetic agents:** May lead to additive pressor effects.
### Monitoring
* Monitor for rebound congestion (increased nasal stuffiness after the medication wears off).
* Monitor blood pressure and heart rate, especially in patients with pre-existing cardiovascular conditions.
### Clinical Pearls
* Limit use to 3 days to prevent rhinitis medicamentosa.
* Instruct patients on proper administration technique (e.g., tilting head slightly forward, inserting nozzle, sniffing gently).
* If symptoms persist beyond 3 days or worsen, advise patient to discontinue and consult a healthcare provider.
