# Oxymetazoline Nasal Spray (e.g., Sinex)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergies, or sinusitis.
## Adult Dosing
* **Spray:** Instill 2 to 3 sprays in each nostril every 10 to 12 hours as needed.
* **Maximum:** Do not exceed 2 doses in a 24-hour period.
* **Duration:** Limit use to a maximum of 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Ages 6 years and older:** Instill 2 to 3 sprays in each nostril every 10 to 12 hours as needed.
* **Ages under 6 years:** Safety and efficacy have not been established. Use should be under the guidance of a healthcare provider.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use in patients with narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Less Common:** Increased blood pressure, palpitations, insomnia, nervousness, headache, dizziness.
* **Serious (rare with topical use):** Rebound congestion (rhinitis medicamentosa) with prolonged or excessive use.
## Key Drug Interactions
* **MAO Inhibitors & within 14 days of stopping MAOIs:** Potentially hypertensive crisis.
* **Tricyclic Antidepressants:** Potentially increased pressor response.
* **Beta-blockers:** May potentiate pressor effects.
## Monitoring
* Monitor for signs and symptoms of rebound congestion (worsening congestion, nasal stuffiness) if used for more than 3 days.
* Monitor blood pressure and heart rate in patients with pre-existing cardiovascular conditions, especially with frequent or prolonged use.
## Clinical Pearls
* Instruct patients to use only as directed and not to exceed the recommended dose or frequency.
* Emphasize the importance of limiting use to 3 days to avoid rhinitis medicamentosa.
* Advise patients to blow their nose gently after application.
* Store at room temperature.
