# Sinex (oxymetazoline)
## Overview
Sinex is a nasal decongestant spray containing oxymetazoline, an alpha-adrenergic agonist that causes vasoconstriction of nasal blood vessels, reducing swelling and congestion.
## Primary Indications
* Nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** Prime the pump before first use. Spray 1-2 sprays into each nostril every 10-12 hours as needed.
* **Maximum:** Do not use more than 2 sprays per nostril in a 24-hour period. Do not use for more than 3 consecutive days.
## Pediatric Dosing
* **Children 6 years and older:** 1-2 sprays into each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended. Consult a pediatrician.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, but caution is advised.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken MAO inhibitors within the past 14 days.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Serious:** Rhinitis medicamentosa (rebound congestion with prolonged use), systemic effects (hypertension, palpitations, dizziness, headache) especially with overuse.
## Key Drug Interactions
* **MAO Inhibitors (e.g., phenelzine, tranylcypromine):** Potentiated hypertensive crisis. Avoid concurrent use.
* **Sympathomimetic Agents (e.g., pseudoephedrine, phenylephrine):** Potential for additive pressor effects. Use with caution.
* **Tricyclic Antidepressants (e.g., amitriptyline, imipramine):** May potentiate cardiovascular effects. Use with caution.
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged use (beyond 3 days).
* Assess for systemic side effects, particularly in patients with cardiovascular disease or those using other sympathomimetic agents.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* Prime the pump before each use, especially if it has not been used recently.
* Wash hands after administration.
* Store at room temperature.
