# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that acts as a nasal decongestant.
## Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
**Nasal Spray:**
* Instill 2 or 3 sprays in each nostril every 10-12 hours as needed.
* Do not exceed 2 doses in a 24-hour period.
* **Maximum duration of use is 3 days.** Rebound congestion (rhinitis medicamentosa) can occur with prolonged use.
## Pediatric Dosing
**Nasal Spray:**
* **Children 6-12 years:** Instill 2 or 3 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period. Maximum duration of use is 3 days.
* **Children under 6 years:** Not recommended. Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment. However, caution is advised in patients with cardiovascular disease.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients who have undergone transsphenoidal hypophysectomy or nasal surgery with dura mater exposure.
* Concurrent use or recent discontinuation of monoamine oxidase inhibitors (MAOIs).
## Adverse Effects
**Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
**Less Common/Serious:**
* **Rhinitis Medicamentosa (Rebound Congestion):** Occurs with prolonged or excessive use.
* **Systemic Effects:** Nervousness, restlessness, insomnia, dizziness, headache, palpitations, increased blood pressure (especially in sensitive individuals or with overuse).
* **Ocular:** Blurred vision, eye irritation.
## Key Drug Interactions
* **MAOIs (and within 14 days of stopping):** May potentiate hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate hypertensive effects.
* **Alpha-blockers:** May antagonize the pressor effects of oxymetazoline.
* **Beta-blockers:** May increase the risk of systemic vasoconstriction and reflex bradycardia.
## Monitoring
* Assess for symptom relief.
* Monitor for signs of rebound congestion with prolonged use.
* Monitor blood pressure, especially in patients with hypertension or cardiovascular disease, or with excessive use.
## Clinical Pearls
* Limit use to 3 consecutive days to prevent rhinitis medicamentosa.
* If symptoms persist after 3 days, discontinue use and consult a healthcare provider.
* Advise patients to use only as directed and to avoid spraying in eyes.
* Store at room temperature.
