## Sinex (Oxymetazoline)
### Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
### Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
### Adult Dosing
**Nasal Spray:**
* **Dose:** 2 sprays in each nostril.
* **Frequency:** Every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days.
### Pediatric Dosing
* **Children under 6 years:** Do not use without consulting a healthcare provider.
* **Children 6 years and older:**
* **Nasal Spray:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days.
### Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment. However, caution is advised in patients with cardiovascular disease.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients who have undergone transsphenoidal hypophysectomy or other transnasal surgery where the dura mater may have been exposed.
### Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, nervousness, insomnia, palpitations, hypertension.
### Key Drug Interactions
* **MAO Inhibitors:** Concomitant use may cause a hypertensive crisis. Oxymetazoline should not be used within 14 days of discontinuing MAO inhibitors.
* **Beta-blockers:** May potentiate the pressor effect of oxymetazoline.
* **Tricyclic Antidepressants:** May potentiate the pressor effect of oxymetazoline.
### Monitoring
* Monitor for signs of rebound congestion or tachyphylaxis with prolonged or frequent use.
* Monitor blood pressure in patients with pre-existing cardiovascular conditions.
### Clinical Pearls
* Limit use to a maximum of 3 consecutive days to avoid rebound congestion.
* If congestion persists or worsens, discontinue use and consult a healthcare provider.
* Store at room temperature.
