# Sinex (oxymetazoline) Nasal Spray
## Overview
Sinex is a topical decongestant nasal spray containing oxymetazoline, an alpha-adrenergic agonist. It causes vasoconstriction of blood vessels in the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **2 sprays in each nostril.**
* **No more frequently than every 10-12 hours.**
* **Do not exceed 2 doses in a 24-hour period.**
* **Limit use to a maximum of 3 consecutive days.**
## Pediatric Dosing
* **Ages 6 to under 12 years:** 1 to 2 sprays in each nostril. No more frequently than every 10-12 hours. Do not exceed 2 doses in a 24-hour period. Limit use to a maximum of 3 consecutive days.
* **Ages under 6 years:** Not recommended. Safety and efficacy have not been established. Consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically needed in adults or pediatrics (ages 6-12) based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use in patients receiving MAO inhibitors or within 2 weeks of discontinuing MAO inhibitors due to risk of hypertensive crisis.
## Adverse Effects
* **Common:** Nasal irritation, dryness, burning, sneezing.
* **Less Common:** Rebound congestion (rhinitis medicamentosa) with prolonged or excessive use, systemic effects (e.g., increased blood pressure, palpitations, insomnia, nervousness) especially with overuse or in susceptible individuals.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use or recent discontinuation can lead to hypertensive crisis.
* **Beta-blockers:** May mask the cardiovascular effects of oxymetazoline.
* **Tricyclic Antidepressants:** Potential for additive pressor effects.
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged use.
* Monitor blood pressure and heart rate in patients with cardiovascular disease or in those experiencing systemic symptoms.
## Clinical Pearls
* Limit use to 3 consecutive days to prevent rebound congestion.
* If congestion persists after 3 days, discontinue use and consult a healthcare professional.
* Ensure proper nasal spray technique to avoid aspiration or excessive systemic absorption.
* Store at room temperature.
