# Oxymetazoline Nasal Spray
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.

## Pediatric Dosing
* **Ages 6 to under 12 years:** Apply 1 to 2 sprays in each nostril every 10-12 hours as needed. Maximum 2 applications per day.
* **Under 6 years:** Not recommended.
## Dose Adjustments
No specific dose adjustments are required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients receiving monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping MAOIs.
## Adverse Effects
* **Common:** Nasal burning or stinging, sneezing, dry nose, temporary blurred vision, headache.
* **Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, cardiovascular effects (hypertension, tachycardia, palpitations), CNS stimulation, insomnia.
## Key Drug Interactions
* **MAOIs and tricyclic antidepressants (TCAs):** Concurrent use or use within 14 days of MAOIs can potentiate the hypertensive effects of oxymetazoline. While not typically documented with TCAs, caution is advised due to potential adrenergic effects.
## Monitoring
* Monitor for signs of rebound congestion, especially with use beyond 3 consecutive days.
* Monitor blood pressure and heart rate in patients with pre-existing cardiovascular conditions.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* If nasal congestion persists, consider alternative treatments such as intranasal corticosteroids or oral antihistamines.
* Educate patients on proper nasal spray technique to ensure optimal delivery and minimize systemic absorption.
