# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, providing temporary relief of nasal congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Sinex Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Sinex SE Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Use:** Do not exceed 2 doses in a 24-hour period. Treatment should not exceed 3 consecutive days to avoid rebound congestion.
## Pediatric Dosing
* **Children 6-11 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Use is not established; consult a pediatrician.
* **Maximum Use:** Do not exceed 2 doses in a 24-hour period. Treatment should not exceed 3 consecutive days.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, as systemic absorption is minimal with topical use.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken MAO inhibitors within the last 14 days.
## Adverse Effects
* **Common:** Nasal discomfort, stinging, burning, dryness, sneezing.
* **Less Common:** Rebound congestion (rhinitis medicamentosa), systemic effects (hypertension, arrhythmias, headache, nervousness, insomnia) with excessive use or absorption.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concurrent use can potentiate severe hypertension. Avoid use within 14 days of MAOI discontinuation.
* **Other Sympathomimetics:** May increase the risk of cardiovascular side effects.
## Monitoring
* Monitor for signs of rebound congestion (worsening nasal congestion after initial relief) with prolonged or frequent use.
* Monitor blood pressure in patients with pre-existing cardiovascular conditions, especially with signs of systemic absorption.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* If congestion persists after 3 days, discontinue use and consult a healthcare provider.
* Prime the pump before first use by actuating it into the air until a fine mist is produced.
