# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* Nasal Spray: 2 sprays in each nostril every 10-12 hours as needed.
* Maximum: Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* Children 6 years and older: 1-2 sprays in each nostril every 10-12 hours as needed.
* Children under 6 years: Safety and efficacy not established. Use is generally not recommended.
## Dose Adjustments
No dose adjustments are typically needed for renal or hepatic impairment, but caution is advised in patients with cardiovascular disease or uncontrolled hypertension.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken MAO inhibitors within the past 14 days.
* Narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, rebound congestion (rhinitis medicamentosa) with prolonged use.
* **Less Common:** Headache, dizziness, nausea, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use can lead to hypertensive crisis. Avoid use within 14 days of MAO inhibitor discontinuation.
* **Beta-blockers:** May mask reflex tachycardia.
* **Tricyclic Antidepressants:** May potentiate pressor effects.
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged use (typically > 3 days).
* Monitor blood pressure and heart rate, especially in patients with cardiovascular disease or hypertension.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* If congestion does not improve or worsens, discontinue use and consult a healthcare provider.
* Prime the pump before first use by spraying into the air until a fine mist appears.
* Clean the nasal applicator after each use.
