## Sinex (Oxymetazoline)
### Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of nasal mucosa, thereby reducing swelling and congestion.
### Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
### Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
### Pediatric Dosing
* **Ages 6-11 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days.
* **Ages less than 6 years:** Safety and efficacy are not established. Consult a pediatrician.
### Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment. However, caution is advised in patients with cardiovascular conditions or hypertension due to potential systemic absorption.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients using MAO inhibitors or within 14 days of discontinuing MAO inhibitors.
### Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, insomnia, palpitations, increased blood pressure. Systemic absorption can lead to cardiovascular effects.
### Key Drug Interactions
* **MAO Inhibitors:** Concurrent use can cause a hypertensive crisis. Avoid use in patients taking or having recently stopped MAO inhibitors.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
* **Beta-blockers:** May reduce the body's ability to regulate blood pressure, potentially leading to an exaggerated hypertensive response.
### Monitoring
* Monitor for signs of rebound congestion with prolonged use.
* In patients with cardiovascular disease or hypertension, monitor blood pressure and heart rate, especially with frequent or prolonged use.
### Clinical Pearls
* Limit use to 3 days maximum. If symptoms persist beyond 3 days, discontinue and consult a healthcare provider.
* Instruct patients on proper nasal spray technique to minimize swallowing and systemic absorption.
* Use with caution in patients with hyperthyroidism, diabetes mellitus, glaucoma, or prostatic hypertrophy.
