## Oxymetazoline Nasal Spray
### Overview
Oxymetazoline is a direct-acting sympathomimetic amine that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
### Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
### Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
### Pediatric Dosing
* **Children 6 years and older:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Children under 6 years:** Not recommended; consult a pediatrician.
### Dose Adjustments
No dose adjustments are typically necessary for hepatic or renal impairment, but caution is advised in patients with cardiovascular disease.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients using monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOIs.
### Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing, runny nose.
Less Common/Serious: Rebound congestion (rhinitis medicamentosa) with prolonged use (>3 days), hypertension, cardiac arrhythmias, headache, insomnia, nervousness, nausea.
### Key Drug Interactions
* **MAOIs and other sympathomimetics:** Risk of hypertensive crisis. Avoid concurrent use.
### Monitoring
* Monitor for signs of rebound congestion or systemic absorption (e.g., increased blood pressure, heart rate).
* Ensure the product is not used for longer than 3 consecutive days to minimize the risk of rhinitis medicamentosa.
### Clinical Pearls
* Rhinitis medicamentosa (rebound congestion) can occur with prolonged or excessive use, leading to a cycle of dependency. Advise patients to limit use to 3 days.
* Use cautiously in patients with cardiovascular disease (hypertension, coronary artery disease, hyperthyroidism) due to potential for systemic absorption and cardiovascular effects.
