# Oxymetazoline (Sinex)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that causes vasoconstriction. It is available in nasal spray formulations.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** Two sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in 24 hours.
## Pediatric Dosing
* **Children 6 years and older:** Two sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in 24 hours.
* **Children under 6 years:** Safety and efficacy have not been established. Consult with a pediatrician.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or have within the past 14 days taken monoamine oxidase inhibitors (MAOIs).
## Adverse Effects
* **Common:** Nasal discomfort, stinging, burning, dryness, sneezing.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic absorption leading to increased blood pressure, palpitations, nervousness, insomnia.
## Key Drug Interactions
* **MAOIs:** Concomitant use or use within 14 days of MAOIs can lead to a hypertensive crisis.
* **Other sympathomimetic agents:** May potentiate systemic effects like increased blood pressure.
## Monitoring
* Assess for relief of nasal congestion.
* Monitor for signs of rebound congestion (worsening congestion after medication wears off).
* Monitor for systemic effects (e.g., elevated blood pressure, palpitations) with overuse or in susceptible individuals.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to minimize the risk of rebound congestion.
* If nasal congestion persists, consider alternative treatments and consult a healthcare provider.
* Ensure proper technique when administering the nasal spray to maximize efficacy and minimize systemic absorption.
