## Sinex (oxymetazoline)
### Overview
Oxymetazoline is a topical nasal decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of the nasal mucosa.
### Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
### Adult Dosing
* **Nasal Spray:** Administer 2-3 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
### Pediatric Dosing
* **Ages 6-11 years:** Administer 2-3 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Ages under 6 years:** Safety and efficacy have not been established. Consult a pediatrician.
### Dose Adjustments
No dose adjustments are typically required based on hepatic or renal impairment due to minimal systemic absorption.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients taking or who have recently taken MAO inhibitors (within 14 days) due to risk of severe hypertensive crisis.
### Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Rebound Congestion (Rhinitis Medicamentosa):** Occurs with prolonged or excessive use (typically >3 days). This can lead to worsening congestion and dependence on the spray.
* **Systemic Effects (rare):** Headache, dizziness, increased blood pressure, insomnia, nervousness, palpitations.
### Key Drug Interactions
* **MAO Inhibitors:** Concomitant use or use within 14 days of MAO inhibitors can lead to hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effect of oxymetazoline.
* **Alpha-blockers and Beta-blockers:** May potentially blunt the effects of oxymetazoline or lead to additive pressor effects.
### Monitoring
* Monitor for signs and symptoms of rebound congestion (rhinitis medicamentosa).
* Monitor blood pressure in patients with pre-existing hypertension, especially with frequent or prolonged use.
### Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* If congestion persists beyond 3 days, discontinue use and consult a healthcare provider.
* Advise patients to use the lowest effective dose for the shortest duration necessary.
