# Sinex (oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, an alpha-adrenergic agonist that causes vasoconstriction of nasal mucosal blood vessels, providing temporary relief of nasal congestion.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Sinex Original (0.05%):** 2-3 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Sinex SE (0.05%):** 2-3 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Sinex D (0.05%):** 2-3 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
## Pediatric Dosing
* **Sinex Original (0.05%):** Not recommended for children under 6 years of age.
* **Sinex SE (0.05%):** Not recommended for children under 6 years of age.
* **Sinex D (0.05%):** Not recommended for children under 6 years of age.
* For children 6 years and older, follow adult dosing.
## Dose Adjustments
No specific dose adjustments are generally required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Use within 14 days of discontinuing a monoamine oxidase inhibitor (MAOI).
## Adverse Effects
Common: Nasal burning, stinging, sneezing, increased nasal discharge.
Less common: Rebound congestion (rhinitis medicamentosa) with prolonged use, insomnia, dizziness, nervousness, headache, palpitations.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concurrent use or use within 14 days of MAOIs may potentiate the hypertensive effects of oxymetazoline.
* **Tricyclic Antidepressants (TCAs):** May potentiate the hypertensive effects of oxymetazoline.
* **Other Sympathomimetics:** Use with caution due to additive pressor effects.
## Monitoring
* Monitor for signs of rebound congestion with prolonged or frequent use.
* Monitor blood pressure in patients with pre-existing hypertension, especially with frequent use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
* If congestion is not relieved, consult a healthcare professional.
* Avoid spraying in the eyes.
