# Sinex (oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, a topical alpha-adrenergic agonist that causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Intranasal:** 2 sprays in each nostril every 10-12 hours as needed.
* Do not exceed 2 applications in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically needed for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Concurrent use with monoamine oxidase inhibitors (MAOIs) or within 2 weeks of discontinuing MAOIs.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Rebound congestion (rhinitis medicamentosa):** Occurs with prolonged or excessive use (>3 days). Symptoms include worsening congestion upon cessation of the drug.
* **Systemic effects (rare):** Headache, dizziness, palpitations, increased blood pressure, nervousness.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concomitant use can lead to hypertensive crisis. Avoid use within 2 weeks of MAOI discontinuation.
* **Other Sympathomimetic Agents:** May potentiate systemic hypertensive effects.
## Monitoring
* Monitor for signs of rebound congestion with prolonged use.
* Monitor blood pressure in patients with pre-existing cardiovascular conditions, especially with frequent or prolonged use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to minimize the risk of rebound congestion.
* Proper technique: prime the pump before first use, tilt head slightly forward, insert nozzle into nostril, spray while breathing in.
* If congestion persists, consider alternative treatments or consult a healthcare provider.
