# Sinex (oxymetazoline)
## Overview
Sinex is a nasal spray containing oxymetazoline, a decongestant that works by constricting blood vessels in the nasal passages, reducing swelling and congestion.
## Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum duration of use:** Do not exceed 3 days of use.
## Pediatric Dosing
* **Ages 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum duration of use:** Do not exceed 3 days of use.
* **Under 6 years:** Consult a healthcare professional.
## Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment, but caution is advised in patients with significant organ dysfunction due to potential systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken MAO inhibitors within the past 14 days.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, dryness, sneezing.
* **Serious:** Rhinitis medicamentosa (rebound congestion) with prolonged or excessive use, hypertension, nervousness, insomnia, dizziness, palpitations, headache.
## Key Drug Interactions
* **MAO inhibitors:** Concurrent use can lead to hypertensive crisis. Avoid use.
* **Other sympathomimetics:** May potentiate cardiovascular effects (e.g., increased blood pressure, heart rate).
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged use.
* Monitor blood pressure and heart rate, especially in patients with pre-existing cardiovascular conditions.
## Clinical Pearls
* Sinex is intended for short-term relief of nasal congestion. Prolonged use (more than 3 days) can lead to rhinitis medicamentosa, worsening congestion.
* Instruct patients to use the product as directed and to discontinue use if symptoms do not improve or worsen.
* Avoid touching the nasal applicator tip to the nostril to prevent contamination.
