# Sinex (oxymetazoline)
## Overview
Sinex is a nasal decongestant spray containing oxymetazoline, an alpha-adrenergic agonist that causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Sinex Original/Extra Strength:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Sinex Severe:** 2-3 sprays in each nostril every 10-12 hours as needed. Do not exceed 3 doses in a 24-hour period.
## Pediatric Dosing
* **Sinex Original/Extra Strength:**
* Ages 6 years to under 12 years: 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* Under 6 years: Safety and efficacy not established. Consult a pediatrician.
* **Sinex Severe:**
* Ages 12 years and older: 2-3 sprays in each nostril every 10-12 hours as needed. Do not exceed 3 doses in a 24-hour period.
* Under 12 years: Safety and efficacy not established. Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, but use with caution and monitor for systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or have taken MAO inhibitors within the past 14 days.
## Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
Less Common/Serious: Rebound congestion (rhinitis medicamentosa) with prolonged use (>3 days), headache, dizziness, increased blood pressure, palpitations, insomnia, nervousness.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use can lead to hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
* **Other Sympathomimetics:** Potential for additive systemic effects.
## Monitoring
* Monitor for signs and symptoms of rebound congestion with prolonged use.
* Monitor blood pressure and heart rate, especially in patients with cardiovascular disease or those using other sympathomimetic agents.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* Instruct patients to blow their nose before use.
* Prime the pump before first use and after storage by spraying 4 times.
* Store at room temperature away from heat and light.
