# Sinex (Oxymetazoline)
## Overview
Sinex is a nasal decongestant spray containing oxymetazoline, an alpha-adrenergic agonist that causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, sinusitis, allergic rhinitis, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years of age:** 1 to 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Children under 6 years of age:** Safety and efficacy have not been established. Consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically necessary based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or within 2 weeks of stopping a monoamine oxidase inhibitor (MAOI).
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged or excessive use (typically > 3 days). Systemic effects (rare) include increased blood pressure, palpitations, anxiety, nervousness, insomnia.
## Key Drug Interactions
* **MAO Inhibitors (and within 2 weeks of discontinuation):** Potentiates hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate pressor response.
## Monitoring
* Monitor for signs and symptoms of rebound congestion with prolonged use.
* Monitor blood pressure and heart rate in patients with cardiovascular disease or those at risk for systemic absorption.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* Proper administration technique is important: clear the nasal passages before use, shake the container, prime the pump if necessary, tilt head slightly forward, insert nozzle into nostril, and spray while breathing in gently.
