# Oxymetazoline Nasal Spray (Sinex)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of the nasal mucosa, reducing swelling and congestion. It is available as a nasal spray.
## Primary Indications
* Nasal congestion due to the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **2 sprays** in each nostril every **10 to 12 hours**.
* Do not exceed **2 doses** in a 24-hour period.
* Limit use to a maximum of **3 consecutive days** to prevent rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10 to 12 hours. Do not exceed 2 doses in 24 hours. Limit use to 3 consecutive days.
* **Children under 6 years:** Use is generally not recommended due to lack of established dosing and potential for systemic absorption. Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, as systemic absorption is minimal when used as directed.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic effects (e.g., increased blood pressure, palpitations, nervousness, insomnia) especially with overuse or in sensitive individuals.
## Key Drug Interactions
* **MAO inhibitors and tricyclic antidepressants:** Coadministration can potentiate the hypertensive effects of oxymetazoline. Discontinue MAO inhibitors at least 14 days prior to use.
## Monitoring
* Monitor for signs and symptoms of rebound congestion with prolonged use.
* Monitor blood pressure and heart rate in patients with pre-existing cardiovascular conditions, especially with overuse.
## Clinical Pearls
* Limit use to 3 consecutive days to avoid rebound congestion. If symptoms persist, discontinue use and consult a healthcare provider.
* Proper administration technique is important: clear the nasal passages before use, tilt head back slightly, and spray into each nostril.
* Avoid spraying directly on the nasal septum.
* Store at room temperature.
