# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that acts as a nasal decongestant.
## Primary Indications
Relief of nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
Instill 2 to 3 sprays in each nostril every 10 to 12 hours. Do not exceed 2 doses in 24 hours.
## Pediatric Dosing
* **Children 6 to under 12 years:** 2 to 3 sprays in each nostril every 10 to 12 hours. Do not exceed 2 doses in 24 hours.
* **Children under 6 years:** Not recommended.
## Dose Adjustments
No dose adjustments are necessary for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline.
* Patients with narrow-angle glaucoma.
## Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing.
Less Common: Increased blood pressure, insomnia, nervousness, headache, dizziness, palpitations, rebound congestion (rhinitis medicamentosa) with prolonged use.
## Key Drug Interactions
* Monoamine oxidase inhibitors (MAOIs): Concurrent use may lead to hypertensive crisis. Discontinue MAOIs at least 14 days before initiating oxymetazoline.
* Other sympathomimetic agents: May potentiate cardiovascular effects.
## Monitoring
* Monitor for signs of rebound congestion with prolonged or excessive use.
* Monitor blood pressure in patients with cardiovascular disease.
## Clinical Pearls
* Limit use to a maximum of 3 days to avoid rebound congestion.
* Instruct patients to avoid spraying in the eyes.
* If congestion is not relieved, consult a healthcare provider.
