# Oxymetazoline Nasal Spray (Sinex)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, reducing swelling and congestion.
## Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Dosage:** 2 sprays in each nostril.
* **Frequency:** Every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Ages 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed. Do not use for more than 3 consecutive days.
* **Ages under 6 years:** Safety and efficacy not established. Consult a healthcare provider.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, but caution is advised.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common:** Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, dizziness, insomnia, palpitations.
* **Rare/Serious:** Systemic effects (hypertension, arrhythmias) are possible with excessive use or accidental ingestion, especially in children.
## Key Drug Interactions
* **MAO Inhibitors (and within 14 days of stopping):** May cause hypertensive crisis.
* **Other Sympathomimetic Agents:** May potentiate systemic pressor effects.
## Monitoring
* Monitor for signs of rebound congestion with prolonged or frequent use.
* Monitor blood pressure and heart rate if systemic absorption is suspected.
## Clinical Pearls
* Limit use to a maximum of 3 days to prevent rhinitis medicamentosa.
* If congestion persists, consider alternative treatments and consult a healthcare provider.
* Avoid spraying directly on the nasal septum.
* Store at room temperature.
