# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Consult a pediatrician. Safety and efficacy are not established.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment for topical nasal administration.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients using MAO inhibitors or within 2 weeks of discontinuing MAO inhibitors.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, dryness, sneezing, increased nasal discharge, rebound congestion (rhinitis medicamentosa) with prolonged use.
* **Less Common:** Dizziness, headache, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **MAO Inhibitors and Tricyclic Antidepressants:** Concurrent use may potentiate the hypertensive effects of oxymetazoline. Avoid use within 14 days of MAO inhibitor therapy.
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged or frequent use.
* Monitor blood pressure in patients with pre-existing hypertension, especially with frequent use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* If nasal passages are severely blocked, blow nose gently before administration.
* Keep tip clean and replace cap immediately after use.
