## Oxymetazoline Nasal Spray
### Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
### Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
### Adult Dosing

Do not exceed 2 doses in 24 hours.
### Pediatric Dosing
* **Children 6 to under 12 years of age:** 2-3 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Children under 6 years of age:** Safety and efficacy have not been established. Consult a pediatrician.
### Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment when used as directed.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use in patients with narrow-angle glaucoma.
### Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic effects (hypertension, nervousness, insomnia, palpitations, headache) especially with overuse or in sensitive individuals.
### Key Drug Interactions
* Use with caution in patients taking monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOIs due to potential for hypertensive crisis.
* May interact with other sympathomimetic agents, potentially increasing cardiovascular effects.
### Monitoring
* Monitor for signs of rebound congestion or rhinitis medicamentosa with prolonged use (beyond 3 days).
* Monitor blood pressure and heart rate in patients with cardiovascular disease or those experiencing systemic symptoms.
### Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* Blow the nose gently before administering the spray.
* If more than one nasal medication is being used, administer oxymetazoline last to allow it to remain in the nasal passages.
