# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal vasculature, reducing swelling and congestion.
## Primary Indications
Symptomatic relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
Instill 2 to 3 sprays in each nostril every 10-12 hours as needed.
Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
**Children 6-11 years:**
Instill 2 to 3 sprays in each nostril every 10-12 hours as needed.
Do not exceed 2 doses in a 24-hour period.
**Children under 6 years:**
Not recommended. Consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically needed for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
Hypersensitivity to oxymetazoline or any component of the formulation.
Patients with narrow-angle glaucoma.
## Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing.
Less Common: Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, dizziness, insomnia.
## Key Drug Interactions
MAO inhibitors and tricyclic antidepressants: May potentiate the pressor effect of oxymetazoline. Concurrent use should be avoided.
## Monitoring
Monitor for signs of rebound congestion, especially with use beyond 3 days.
Assess for systemic side effects (e.g., increased blood pressure) if significant systemic absorption is suspected, though this is rare.
## Clinical Pearls
Use should be limited to a maximum of 3 consecutive days to prevent rebound congestion.
If symptoms persist, discontinue use and consult a healthcare provider.
