# Oxymetazoline Nasal Spray
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of nasal vasculature. This reduces swelling and congestion in the nasal passages.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Standard:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days. Prolonged use can lead to rhinitis medicamentosa (rebound congestion).
## Pediatric Dosing
* **Children 6 to under 12 years:** 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Safety and efficacy have not been established. Consult with a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients using monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOIs.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common/Serious:** Rhinitis medicamentosa (rebound congestion with overuse), systemic cardiovascular effects (hypertension, tachycardia, arrhythmias) especially with overuse or in susceptible individuals.
## Key Drug Interactions
* **MAOIs:** Potentially severe hypertensive crisis. Avoid concurrent use and within 14 days of MAOI discontinuation.
* **Beta-blockers:** May mask reflex tachycardia.
* **Tricyclic Antidepressants (TCAs):** May potentiate pressor effects.
## Monitoring
* Monitor for signs of rhinitis medicamentosa (worsening congestion, dependence on the spray).
* Monitor blood pressure and heart rate if systemic absorption is suspected (e.g., with excessive use).
## Clinical Pearls
* Use only as directed and for the shortest duration necessary.
* Avoid spraying directly onto the nasal septum.
* Dispose of the product 3 months after opening.
* If symptoms persist or worsen, discontinue use and consult a healthcare provider.
