# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction. In nasal spray formulations, it reduces nasal congestion.
## Primary Indications
* Nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray (0.05%):** Instill 2 or 3 sprays in each nostril every 10 to 12 hours as needed.
* **Maximum:** Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** Instill 2 or 3 sprays in each nostril every 10 to 12 hours as needed.
* **Children under 6 years:** Use is not established. Consult with a pediatrician.
## Dose Adjustments
No dose adjustments are typically necessary for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients using MAO inhibitors or who have used them within the last 14 days.
## Adverse Effects
* **Common:** Nasal burning, stinging, irritation, dryness, sneezing.
* **Rebound congestion (Rhinitis medicamentosa):** Occurs with prolonged or excessive use (beyond 3 days). Symptoms include worsening nasal congestion, which may lead to dependence.
* **Systemic effects (rare but possible with overuse):** Headache, nervousness, dizziness, increased blood pressure, insomnia, palpitations.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concomitant use or use within 14 days of MAOIs can cause a hypertensive crisis.
* **Tricyclic Antidepressants (TCAs) and other sympathomimetics:** May potentiate the pressor effects of oxymetazoline.
## Monitoring
* Monitor for signs of rebound congestion with prolonged use.
* Monitor blood pressure in patients with pre-existing hypertension or those using other vasoconstrictive medications, especially with excessive use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* If congestion does not improve, advise patients to discontinue use and consult a healthcare provider.
* Ensure the nasal spray is administered upright to prevent clogging.
