# Sinex (Oxymetazoline)
## Overview
Sinex is a topical nasal decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergies, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Daily Dose:** Do not exceed 2 doses in a 24-hour period.
* **Duration of Use:** Limit use to a maximum of 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended without physician guidance.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment. However, caution is advised in patients with cardiovascular disease or hypertension.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use within 14 days of MAOI therapy.
## Adverse Effects
* **Common:** Nasal discomfort, stinging, burning, dryness, sneezing.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic absorption leading to increased blood pressure, palpitations, nervousness, insomnia.
## Key Drug Interactions
* **MAO Inhibitors (and within 14 days of stopping):** Potentially severe hypertensive crisis.
* **Beta-blockers:** May potentiate the hypertensive effects.
* **Tricyclic Antidepressants:** May potentiate the vasoconstrictive effects.
## Monitoring
* Monitor for signs of rebound congestion with prolonged or excessive use.
* Monitor blood pressure and heart rate in patients with cardiovascular risk factors, especially with frequent or prolonged use.
## Clinical Pearls
* Sinex provides temporary relief of nasal congestion. It does not treat the underlying cause.
* Limit use to 3 days to prevent rhinitis medicamentosa.
* If symptoms persist or worsen, discontinue use and consult a healthcare provider.
* Blow the nose gently before administration to ensure proper product delivery.
* Store at room temperature.
