# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of nasal mucosa. This reduces swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended without physician guidance. Safety and efficacy are not established.
## Dose Adjustments
* No dose adjustments are typically required for hepatic or renal impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Use with caution in patients with cardiovascular disease, hypertension, hyperthyroidism, diabetes mellitus, or prostatic hypertrophy.
## Adverse Effects
* **Common:** Nasal burning, stinging, irritation, dryness. Sneezing.
* **Less Common/Serious:** Rhinitis medicamentosa (rebound congestion) with prolonged or excessive use. Palpitations, increased blood pressure, insomnia, nervousness.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concurrent use may potentiate the pressor effects of oxymetazoline. Avoid concurrent use and for 14 days after discontinuing MAOIs.
* **Beta-blockers:** May mask reflex tachycardia.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa), especially with prolonged use (beyond 3 days).
* Monitor blood pressure and heart rate in patients with pre-existing cardiovascular conditions.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* If symptoms persist after 3 days, discontinue use and consult a healthcare provider.
* Prime the pump before first use or if it has not been used for several days.
* Avoid spraying in eyes.
