# Oxymetazoline Nasal Spray (Sinex)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal vasculature, reducing mucosal swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Standard Strength (0.05%):** Prime pump before first use. Administer 2 sprays into each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Extra Strength (0.05%):** Administer 2-3 sprays into each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Children 6-12 years:** Standard Strength (0.05%) is typically used. Administer 2 sprays into each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Children under 6 years:** Use is generally not recommended due to potential for increased systemic absorption and adverse effects. Consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken MAO inhibitors within the past 14 days (risk of hypertensive crisis).
* Risk of rebound congestion with prolonged use.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common:** Headache, nausea, dizziness.
* **Serious/Overuse Related:** Rebound nasal congestion (rhinitis medicamentosa), nasal ulceration, epistaxis, systemic hypertension, tachycardia, CNS depression, seizures, insomnia.
## Key Drug Interactions
* **MAO Inhibitors:** Concomitant use is contraindicated due to the risk of hypertensive crisis.
* **Other Sympathomimetics:** Use with caution; may potentiate systemic pressor effects.
* **Beta-blockers:** May mask reflex tachycardia.
## Monitoring
* Monitor for signs of rebound congestion and nasal irritation with prolonged use.
* Observe for systemic side effects (e.g., elevated blood pressure, increased heart rate), especially in patients with cardiovascular disease or if excessive use is suspected.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* Proper technique is important: tilt head forward slightly, insert nozzle into nostril, and spray while breathing in gently.
* Priming the pump is necessary for consistent dosing.
