# Sinex (oxymetazoline)
## Overview
Sinex is a topical nasal decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal vasculature, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergies, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Consult a pediatrician. Safety and efficacy not established.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Use with caution in patients with cardiovascular disease, hypertension, hyperthyroidism, diabetes, or prostatic hypertrophy.
## Adverse Effects
* **Common:** Nasal burning or stinging, sneezing, increased nasal discharge.
* **Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic absorption leading to increased blood pressure, heart rate changes, headache, nervousness, insomnia.
## Key Drug Interactions
* **MAO Inhibitors:** Concomitant use may lead to hypertensive crisis. Avoid concurrent use and for 14 days after discontinuing MAO inhibitors.
* **Other Sympathomimetics:** May potentiate cardiovascular effects.
## Monitoring
* Monitor for signs of systemic absorption, particularly in patients with pre-existing cardiovascular conditions.
* Assess for rebound congestion if used beyond the recommended duration.
## Clinical Pearls
* Limit use to a maximum of 3 days.
* Prime the pump before first use and if it has not been used for several days.
* Instruct patients to blow their nose before administration.
* Avoid spraying in the eyes.
