# Oxymetazoline Nasal Spray (Sinex)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical vasoconstrictor. It reduces swelling of the nasal mucosa, thereby decreasing nasal congestion.
## Primary Indications
* Relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **2 sprays in each nostril.**
* May be repeated every 10-12 hours as needed.
* **Do not exceed 2 doses in 24 hours.**
## Pediatric Dosing
* **Children 6 to under 12 years of age:** 1-2 sprays in each nostril. May repeat every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Children under 6 years of age:** Consult a healthcare provider.
## Dose Adjustments
* No specific dose adjustments are generally recommended for renal or hepatic impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or within 14 days of stopping a monoamine oxidase inhibitor (MAOI).
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Less Common:** Rhinitis, insomnia, nervousness, increased blood pressure, palpitations, rebound congestion (rhinitis medicamentosa) with prolonged use.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concomitant use may lead to hypertensive crisis. Avoid use within 14 days of MAOI therapy.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
* **Other Sympathomimetic Agents:** May lead to additive sympathomimetic effects.
## Monitoring
* Monitor for rebound congestion with prolonged or excessive use.
* Monitor blood pressure in patients with underlying cardiovascular conditions.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
* If symptoms persist after 3 days, advise the patient to discontinue use and consult a healthcare provider.
* Use with caution in patients with heart disease, hypertension, diabetes, thyroid disease, or enlarged prostate.
