# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
Administer 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in 24 hours.
## Pediatric Dosing
* **6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in 24 hours.
* **Under 6 years:** Not recommended. Consult with a pediatrician.
## Dose Adjustments
No specific dose adjustments are generally required for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
## Adverse Effects
Common: Nasal irritation, burning, stinging, dryness.
Less common: Rhinitis medicamentosa (rebound congestion with prolonged use), headache, dizziness, increased blood pressure, insomnia, nervousness.
## Key Drug Interactions
Concurrent use with monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping MAOIs may cause a hypertensive crisis. Use with other sympathomimetic agents may potentiate cardiovascular effects.
## Monitoring
Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged or excessive use. Monitor blood pressure in patients with pre-existing cardiovascular conditions.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to avoid rhinitis medicamentosa.
* If symptoms persist, discontinue use and consult a healthcare provider.
* Ensure proper administration technique to maximize efficacy and minimize systemic absorption.
