# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 doses in a 24-hour period.
* **Duration:** Limit use to a maximum of 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Use is generally not recommended unless directed by a healthcare provider. Consult product labeling for specific age restrictions and formulations.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment for topical nasal application.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, irritation. Sneezing.
* **Rebound Congestion (Rhinitis Medicamentosa):** With prolonged or excessive use (>3 days), can lead to chronic nasal congestion that is difficult to treat.
* **Systemic Effects (rare with topical use, but possible with overuse or in sensitive individuals):** Increased blood pressure, headache, nervousness, insomnia, palpitations.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs) & within 14 days of MAOI use:** May potentiate hypertensive effects.
* **Tricyclic Antidepressants (TCAs):** May potentiate hypertensive effects.
* **Other Sympathomimetic Agents:** May increase the risk of systemic side effects.
## Monitoring
* Monitor for signs of rebound congestion (worsening congestion after initial relief, often occurring with prolonged use).
* In patients with cardiovascular disease or hypertension, monitor for systemic effects such as increased blood pressure or heart rate, though rare with appropriate topical use.
## Clinical Pearls
* Emphasize limiting use to 3 days to prevent rhinitis medicamentosa.
* Advise patients to blow their nose gently before application.
* Tilt head slightly forward during administration.
* If congestion persists after 3 days, patients should consult a healthcare provider.
