# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 doses in a 24-hour period.
## Pediatric Dosing
* **Children 6 years and older:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended.
## Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or within 14 days of stopping monoamine oxidase inhibitors (MAOIs).
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, sneezing, increased nasal discharge.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic effects (hypertension, palpitations, insomnia, nervousness) particularly with overuse or in sensitive individuals.
## Key Drug Interactions
* **MAOIs and within 14 days of discontinuation:** Risk of hypertensive crisis.
* **Other sympathomimetic agents:** Potentially increased systemic sympathomimetic effects.
## Monitoring
* Monitor for effectiveness in relieving nasal congestion.
* Assess for signs of rebound congestion or systemic side effects.
* Advise patients on duration of use to avoid rhinitis medicamentosa.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* Administer as directed to ensure proper nasal deposition.
* Inform patients to discontinue use and consult a healthcare provider if symptoms persist or worsen.
