# Oxymetazoline (Sinex)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of nasal mucosal blood vessels. This reduces swelling and congestion, providing temporary relief from nasal stuffiness.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** Typically administered as 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications per nostril in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** Typically administered as 1 to 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications per nostril in a 24-hour period.
* **Children under 6 years:** Not recommended; consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use within 14 days of discontinuing a monoamine oxidase inhibitor (MAOI).
## Adverse Effects
* **Common:** Nasal burning, stinging, irritation, dryness. Sneezing. Rebound congestion (rhinitis medicamentosa) with prolonged or excessive use.
* **Less Common:** Increased blood pressure, palpitations, nervousness, insomnia.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concomitant use may potentiate the pressor effects of oxymetazoline. Avoid use within 14 days of MAOI discontinuation.
* **Beta-blockers:** May potentiate the hypertensive effects.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects.
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged use (typically defined as more than 3-5 days of continuous use).
* Monitor blood pressure, especially in patients with pre-existing cardiovascular conditions or those using other sympathomimetic agents.
## Clinical Pearls
* Limit use to a maximum of 3-5 consecutive days to minimize the risk of rhinitis medicamentosa.
* If symptoms persist, discontinue oxymetazoline and consult a healthcare provider.
* Ensure the spray nozzle is clean before use.
